There's a minor bug with primed nades which must be an etmain thing: If you do +scores while you still hold them in the hand, you can't hear the ticks while the scoreboard is still open. Though you can hear the ticks again when closing the scoreboard.

I'm not sure, but I thought there were more situations where you can't hear the ticks, maybe someone else can help...?

It is an etmain bug, and it's been a known problem for a while, but it'll be a pain to fix. The reason it happens is because the HUD code is responsible for making the tick sounds (so that the animation on the weapon icon matches the sound effects.)
