If the frame is opened up(a gap where his cheek is), he could just bite one side, and it would look like it pierced his cheek...

with MUCH, MUCH pain. (the human body can do much more things than u can imagine..)
but still, its photoshop, or an accident. no one is that stupid to stick something like that to his face.

That sounds like a bit on airplane2, where they are talking to each other through using screens/cams from bridge to loading area...

Captain: WHAT, REALLY?
Other: Yes, we need you on the deck right now
*Panel with screen on it opens and captain walks in*
Captain: Yes, Who put them in charge!

(basically, it was just a window in the door
