it puts an advertisement line in your campaign...
but you can open up the pk3, edit the txt campaign file and remove it

in the loading screen it just says that you created this campaign with highboy's campaign creator. nothing more nothing less. spyware not included

If you know how to do that, why even go there to have it make a campaign? I think the sites for total noobs, becuase it isn't that hard.
